TYPES OF SERVICES PROVIDED:

  • Day-of-referral contact, safety assessment, and “triage” of immediate needs.

  • Assistance with shelter, food, clothing, and supplies necessary for family life.

  • Therapeutic intervention and support, including individual and family counseling.

  • Assistance with developmental disabilities resources.

  • Professional case management.

  • Assistance and advocacy with other resources and systems.

  • Transportation assistance.

  • Collateral contacts.

  • Supervised visitation in the community, in the family home, at The Guadalupe House, or at DHS.

  • Planned and supervised reunification.

  • Parenting education.

  • Ongoing safety assessment.

  • Monthly progress reports, court reports and appearances.

  • 24-hour emergency on-call services and crisis management.

 

SERVICE OBJECTIVES:

  • Preservation of the family unit.

  • Prevention of child abuse and neglect.

  • Development of community-based, neighborhood-centered support and assistance service delivery systems.

  • Establishment or reinforcement of support systems (extended family, neighbors, community) that will continue when professional assistance has terminated.

  • Quality visitation between children and parents

MEASURABLE SERVICE OUTCOMES:

  • Reduced incidence of out-of-home placement.

  • Reduced stay in out-of-home placement.

  • Reduced incidence of abuse or neglect.

  • Increased family functioning as determined by completing case plan objectives.

  • Reduced recidivism of families in to the social service system.

 

SERVICE CONTRACT COMPONENTS:

  1. Home-Based Services

  1. Home-Based Intervention Services Program provides the intensive family contact and provision of care necessary for the family to succeed.

  2. Our agency is available on a twenty-four-hour basis to the families we serve and to the Department of Human Services needing to make a referral.

  3. Monthly written progress reports are provided to the county department, the G.A.L., and the Court as necessary.

  4. Language interpreters will be contracted as needed.

  5. Home-based Intervention Services provides up to one year of after-care to families after case closure, at no cost to the Department or the family. 

  1. Visitation Services

  1. The current contract with Denver Department of Human Services includes visitation supervision at low, moderate, and therapeutic levels. In addition, visits may move from fully supervised to intermittent supervision based on the progress of visits and agreement between DDHS, the GAL and the court for a decrease in supervision.

  2. Individual visitation documentation reports are to be provided to the county department within seven days of a visit. Reports are submitted to the Visitation Program Director within five days of the visit to allow for review of reports and for submission to DART (county database). 

  3. A 30-day Visitation Assessment is due within 30 days of the start of visitation services. Assessments are then updated every 60-90 days thereafter, or as necessary if a reduction or increase in supervision is determined appropriate by the visitation supervisor.
